FCRA Background Check Disclosure & Authorization

Luminex Janitorial Services may obtain a consumer report and/or investigative consumer report from a consumer reporting agency for employment purposes, including evaluating eligibility for employment, assignment, retention, reassignment, or promotion.

Stand-alone disclosure: This document is intended to be separate from the employment application. It explains that a background report may be obtained and used for employment-related decisions.

The report may include legally permissible information regarding identity verification, criminal history, employment history, education history, driving record when job-related, sanctions or exclusion checks when relevant, and other information permitted by law.

By signing below, I authorize Luminex Janitorial Services and its authorized screening provider to obtain and use consumer reports and/or investigative consumer reports for employment purposes, subject to applicable law.

I understand that I may request information about the nature and scope of any investigative consumer report and that I have rights under the Fair Credit Reporting Act.
